Job summary

Mercy House Living Centers is seeking a Housing Navigator to support shelter residents with needs assessments, housing plans, referrals, and advocacy to reach housing stability.

The role requires creating individualized service plans, coordinating with external agencies, and applying trauma-informed care and harm reduction in daily interactions. A strong ability to manage a caseload and maintain accurate files is essential.

Qualifications

• Bachelor's degree in Human Services or related field preferred.
• At least 1 year of experience working with homeless individuals and mental health preferred.
• Ability to lift up to 50 pounds and reliable transportation is required.

Responsibilities

• Maintain a caseload of 22-30 individuals.
• Meet with participants as needed or instructed by supervisor.
• Conduct navigation assessment within seven days of entering shelter.
• Assist clients in obtaining documents to secure housing (ID, birth certificate, income verification).
• Provide information referrals, crisis intervention, and assistance with permanent housing placement.
• Develop an Individual Service Plan with short- and long-term goals.
• Input data into HMIS and update progress snapshots.
