Job summary

Volunteers of America Southeast Louisiana Inc seeks a NS PSH Housing Specialist- CCII to provide care coordination for persons served in the community. You will develop and coordinate housing resources and supportive services for individuals who are homeless and disabled under the supervision of Housing Services.

The role requires documenting outcomes, preparing reports, and maintaining accurate electronic records while advocating for program participants.

Qualifications

• Bachelor's Degree and one year experience working with individuals who have a disabling condition in a community setting.
• CPR/First Aid Instructor Certification is required upon hire or within brief time of assuming position duties.
• Excellent verbal and written communication skills; ability to work in a team.

Responsibilities

• Provide care coordination services to persons served and ensure quality outcomes.
