Job summary

Volunteers of America Southeast Louisiana is seeking an NS PSH Housing Specialist- CCII in New Orleans, LA. This role reports to the Housing Supervisor and delivers care coordination for individuals who are homeless and disabled, coordinating housing resources and supportive services in the community.

The position emphasizes documentation, outcome-focused service plans, and collaboration with other providers, with home visits twice a month, weekly contact, and adherence to accreditation

Qualifications

• Bachelor's Degree and one year experience working with individuals who have a disabling condition in a community setting.
• CPR/First Aid Instructor Certification is required upon hire or within brief time of assuming position duties.
• Excellent verbal and written communication skills; ability to work in a team.

Responsibilities

• Prepares required reports.
• Creates and maintains linkages with other service providers to develop resources for the person served.
• Develops and implements individual service plans to ensure persons served receive quality services.
• Performs home visits and off-site case management services; home visits twice a month.
• Contacts participants weekly and documents in the participant’s EHR.
• Documents in participant’s EHR within twenty-four hours of service delivery.
