Job summary

The Salvation Army Northern Division Harbor Light Center in Minneapolis is seeking a Case Manager to provide individualized, strength-based case management for adults pursuing housing, employment, and self-sufficiency.

You will maintain participant records, recruit eligible participants, promote social services, and support community outreach while collaborating with local partners; this is a full-time, on-site role at 1010 Currie Avenue.

Qualifications

• Bachelor's degree in a relevant field; combination of education and experience may be considered.
• State board licensure is required if the degree is in social work.
• Previous experience in social services is required.
• Experience with long-term case management is preferred.
• Must successfully pass all required background checks.
• A valid driver's license is preferred, and candidates must meet MVR requirements.

Responsibilities

• Provide short- and long-term, strength-based, person-centered case management and develop individualized service plans.
• Maintain accurate participant records in case files and databases and document progress.
• Recruit eligible participants for housing programs through outreach and referrals.
• Promote social services and corps programs to increase engagement and accessibility.
• Support and promote The Salvation Army initiatives by collaborating with colleagues and partners.
