Job summary

The People Concern in Los Angeles is seeking a Case Manager for the 1603- Housing Retention program, reporting to the Program Manager. This 100% onsite role is based at Pico Square, Los Angeles, CA 90019, with a Monday–Friday 8:00 a.m. to 4:30 p.m.

Qualifications

High School Diploma or GED required; Bachelor’s degree preferred.
Two years’ experience working with homeless individuals (or equivalent).
Able to work in harm reduction models and transportation of clients.

Responsibilities

Maintain a caseload of approximately 20 high acuity Clients for intensive case management services (up to 40 if low acuity).
Develop relationships with Clients to support independence and health maintenance.
Collaborate with LA County DMH and DHS programs to coordinate treatment efforts.
Engage Clients with harm-reduction, trauma-informed care, and housing-first principles.
Assist in screening, intake, and enrollment and orientation to program policies.
Complete biopsychosocial assessments and functional needs assessments.
Write progress notes and maintain electronic charts.
Coordinate transportation and appointments to improve access to care.
