Job summary

The People Concern is looking for a dedicated Case Manager in Los Angeles to support Clients in achieving housing stability and independence. You will manage a caseload of high-acuity individuals, collaborating with health services and assisting clients in accessing necessary benefits.

This role requires a High School Diploma or GED and a valid CA Driver’s license. The organization provides a wide range of benefits including medical and retirement plans, ensuring a supportive environment for both Clients and staff.

Qualifications

• High School Diploma or GED or Equivalent.
• Valid CA Driver’s license, reliable car, and acceptable driving record.
• Ability to work in a high tolerance Harm Reduction model.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients.
• Develop relationships with Clients to facilitate independence.
• Collaborate with the LA County mental health and health services.
• Assist Clients in applying for and obtaining benefits.
