Job summary

Breaking Ground seeks a Case Manager, Outreach, to provide direct case management for people experiencing homelessness, including housing transition support and coordination with behavioral health services. The role reports to the Team Leader, Outreach and requires conducting assessments, benefits assistance, and service planning for a caseload of about 25 clients.

The position operates a 35-hour work week and emphasizes harm reduction, crisis intervention, and help obtaining permanent housing.

Qualifications

• Bachelor’s Degree or equivalent work experience.
• Two years of experience with homeless adults and substance use issues preferred.
• Experience with harm reduction and motivational interviewing strongly preferred.
• Valid NY/NJ/CT/PA driver’s license with a good driving record required.

Responsibilities

• Provide direct services to a caseload of approximately 25 clients, including assessments and benefits assistance.
• Conduct psychosocial evaluations.
• Connect clients to psychiatric services.
• Connect clients to medical treatment and substance use treatment facilities when applicable.
• Obtain identification, public benefits, and other applicable income sources for clients.
• Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans.
• Prepare all documentation for housing applications.
• Assist and counsel clients in obtaining permanent housing.
• Maintain records in compliance with agency and city/state standards.
• Intervene in crisis situations.
• Escort clients to appointments when needed.
• Provide assistance with self-administration of medication after training.
