Job summary

Breaking Ground is seeking a Case Manager for the Street to Home outreach program in Brooklyn, NY. You will provide direct services to a caseload of clients experiencing homelessness, including assessments, benefits assistance, and housing navigation.

The role emphasizes collaborative planning and trauma-informed care. Responsibilities include psychosocial evaluations, linking clients to psychiatric and medical services, benefits coordination, and crisis intervention.

Qualifications

• Bachelor's degree or equivalent work experience
• Two years of experience working with homeless adults and substance abuse issues preferred
• Experience with harm reduction and motivational interviewing strongly preferred
• Valid NY, NJ, CT, or PA driver’s license with a good driving record

Responsibilities

• Provide direct services to a caseload of approximately 25 clients, including assessments and benefits assistance
• Conduct psychosocial evaluations
• Connect clients to psychiatric services and medical treatment
• Assist in obtaining identification, public benefits, and income sources
• Meet weekly with clients to assess needs and develop service plans
• Prepare documentation for housing applications and housing procurement
• Maintain records in compliance with agency and city/state standards
• Intervene in crisis situations and escort clients to appointments
• Support self-administration of medications following policy
