Job summary

Breaking Ground’s Street to Home program coordinates street outreach teams to connect more than 2,000 individuals to housing and support services annually. The Outreach Coordinator ensures field coverage, supervises Housing Advocates, and applies Trauma-informed, client-centered approaches in the moment.

The role focuses on staffing, data entry compliance, and collaboration with partners, with evening shifts and on-call responsibilities, reporting to the Assistant Director, Programs, Outreach.

Qualifications

• Must hold NYS LMSW license.
• Minimum 3 years supervisory experience.
• 9.58 credentialing within first 3 months of hire.
• Understanding of Housing First and Harm Reduction models.
• Experience with crisis intervention, risk assessments, and incident management.
• Excellent communication and collaboration skills with diverse populations.

Responsibilities

• Oversee street outreach teams and ensure coverage.
• Provide regular supervision meetings for direct reports.
• Implement supervisory strategies and accountability.
• Create and manage shift schedules for continuous field deployment.
• Incorporate team and data feedback into canvassing activities.
• Promote Housing First, Harm Reduction, CPI, and motivational interviewing.
• Ensure contracts and performance targets are met and documented.
• Guide staff in crisis situations and de-escalation.
• Manage Code Blue/Red alerts and on-call coverage as needed.
• Coordinate with community partners and stakeholders.
