Job summary

Wellspring in Louisville, KY is hiring a full-time Supportive Housing Program (SHP) Case Manager to help clients achieve stable housing and recovery goals. You will assess needs, develop individualized service plans, coordinate with medical and social services, and advocate for client access to benefits.

A personal vehicle is required for travel between sites and appointments. The role demands a Bachelor’s degree (or equivalent experience) and emphasizes empathy, collaboration, and compliance

Qualifications

• A personal vehicle is required; traveling between work sites and transporting clients to appointments.
• A Bachelor's Degree is required, but experience and relevant coursework may be an acceptable alternative.
• Preference given to candidates with experience in social services, mental illness services, and homelessness services.

Responsibilities

• Assess client needs and goals, create a service plan, and monitor progress.
• Maintain client records and ensure documentation per HUD, Wellspring policies, and funding/licensing requirements.
• Maintain confidentiality and HIPAA compliance; collaborate with the team on housing, safety, self-sufficiency, and recovery goals.
• Provide life skills coaching and coordinate with medical, employment, education, and community resources.
• Assist clients with access to benefits and crisis intervention as needed.
