Job summary

Responsibilities

• Determines nature of client’s situation by intake and assessment.

• Connects clients to resources (mental health, employment, government assistance, housing).
• Maintains records and ensures HMIS timely updates.
• Monitors planned actions and follows safety procedures.
• Works with diverse teams and maintains client confidentiality.
