Job summary

Wellspring is seeking a full-time SHP Case Manager in Louisville, KY, to support clients in securing stable housing and advancing recovery. The role requires travel between sites, client assessments, service planning, and coordination with medical and community resources.

Qualifications

• Experience in social services, mental illness services, and homeless services preferred.
• Must have reliable personal vehicle for travel between work sites.

Responsibilities

• Assess client needs and goals, create service plans, and connect to resources.
• Maintain client records and document homelessness status and disability per HUD and Wellspring policy.
• Ensure confidentiality and HIPAA adherence.
• Collaborate with team and monitor housing stability and recovery goals.
• Provide life skills coaching and crisis intervention when needed.
• Coordinate with medical, behavioral health, employment, and education services.
