Job summary

Sacramento Native American Health Center, Inc seeks a Project Coordinator to monitor contracts and coordinate with seven counties for the Partners II program. You will ensure program compliance, reporting, and data tracking, including housing-related tasks and hotel placements for CALAIM patients.

The role requires strong communication, Excel proficiency, and the ability to work with multi-disciplinary teams while maintaining confidentiality.

Qualifications

• Strong verbal and written communication.
• Independently manage documentation with accuracy.
• Experience working with diverse teams and stakeholders.
• Knowledge of data reporting and tracking.
• Ability to maintain confidentiality and sensitive information.
• Familiarity with HMIS or related housing systems is desirable.

Responsibilities

• Assist in maintaining Partners II program compliance and deliverables.
• Coordinate with Seven COC County Representatives for program compliance.
• Oversee hotel placements for patients under CALAIM and manage reports.
• Provide regular data updates to internal meetings and stakeholders.
• Prepare and submit accurate program reports and funding documentation.
• Assist in planning events aligned with organizational goals.
• Collaborate with Housing partners in the NorCal Continuum of Care.
• Be active in HMIS and Coordinated Entry processes.
• Propose solutions for complex housing challenges.
• Maintain Housing First and Partners II Housing program compliance.
