Qualifications

• Bachelor's degree in social work/mental health or two years direct mental health services experience.
• Experience with homeless populations and housing stability.
• Knowledge of Fair Housing laws and LA CoC.

Responsibilities

• Verify participant eligibility during enrollment.
• Develop Individualized Care Plans within 30 days of enrollment.
• Assist with housing searches, placements, and tenancy support.
• Advise on rental applications and lease terms; educate on rights.
• Keep documentation updated and enter data into required systems.
