Job summary

Pacific Health Group in California is seeking a Field-based Housing Stability Specialist to support CalAIM Community Supports Housing programs. The role combines field visits, assessments, and care coordination to help members secure and maintain stable housing while connecting them with healthcare and social services.

You will travel locally, build partnerships with housing providers, conduct intake assessments, manage housing deposits, and document in real time while meeting Medi-Cal/CalAIM

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports (preferred).
• Proficient with case management systems and EHR platforms.
• Strong communication, empathy, and cultural competence.
• Excellent organizational and time-management skills.

Responsibilities

• Support housing navigation and deposits as part of CalAIM programs.
• Conduct outreach, intake assessments, and build housing provider partnerships.
• Coordinate housing tenancy, sustaining services, and long-term care plans.
• Travel locally for field visits and member support.
• Document interactions and maintain real-time charts per guidelines.
