Responsibilities

  • Oversee hotel placements for patients under CALAIM and manage reports.
  • Provide regular data updates to internal meetings and stakeholders.
  • Prepare and submit accurate program reports and funding documentation.
  • Assist in planning events aligned with organizational goals.
  • Collaborate with Housing partners in the NorCal Continuum of Care.
  • Be active in HMIS and Coordinated Entry processes.
  • Propose solutions for complex housing challenges.
  • Maintain Housing First and Partners II Housing program compliance.

JOB SUMMARY

Under the general direction of the Supportive Services Center Manager theProject Coordinator is responsible for monitoring contracts and communication with the seven counties of the NorCal COC for the Partners II Program. They will ensure compliance with Partners II program requirements, reporting, and application submissions and data tracking. which includes but not limited to rental payments and landlord engagement to support tenants’ sustainability. This position will solve complex housing issues that impact residents. This position will be involved with the Coordinated Entry and Homeless Management Information system. Additionally, they support Hotel placements, payment and check deliveries for patients in Community Support Program underCalifornia Advancing and Innovating Medi-Cal CalAIM. The position will also be responsible for performing other duties as assigned by the Center Manager. The position requires the ability to communicate effectively and demonstrate strong accounting skills, work well as a member of a multi-disciplinary team and substantial independent judgement and initiative.

JOB DUTIES AND RESPONSIBILITIES
  • Assists in maintaining Partners II program compliance and progress by preparing and reviewing program deliverables,
  • Collaborate with the Seven COC County Representatives to promote successful program initiative compliance
  • Hotel placement of patients under Community Support Program under CALAIM Program. Develop and manage reports and tracking of programs under the oversight of the Center Manager.
  • Delivers regular updates on data, progress, and outcomes at internal meetings and with key stakeholders.
  • Works with departments lead to preparing and submit accurate and timely reports as required by Partners II program agreements and funding sources; along with coordinating placement and payment of patients in hotels under CALAIM Program.
  • Supports the planning and execution of mission-driven events that align with organizational goals and objectives.
  • Collaborates with Housing partners that are part of the NorCal Continuum of Care.
  • Active participation in the Homeless Management and Information System and Coordinated Entry.
  • Problem solves complex housing solutions.
  • Maintains compliance with Housing First Principles.
  • Maintains compliance with the Partners II Housing program as well as Community Support Program.
  • Performs other duties as assigned.

EDUCATION & EXPERIENCE REQUIREMENTS
  • Maintain a valid CA driver license and meet the requirements of SCHC Auto policy.
PREFERRED QUALIFICATIONS
  • Knowledge of Cal Aim (Medi-Cal Transformation) would be desirable.
  • Knowledge of, or experience with HUD is preferred
  • Ability to work in a fast-paced environment.
  • Knowledge of Housing First principles is desired.
  • Knowledge of Coordinated Entry System and HMIS is desired.
  • Knowledge of HOPE Medical Program.
