Lutheran Social Services of Illinois (LSSI) is hiring a full-time Counselor Case Manager for the Homeless Outreach & Prevention Services program in Chicago, Monday through Friday.

Responsibilities include outreach, engagement, and housing-related case management for unhoused individuals, with emphasis on reducing crisis episodes and improving access to housing. Requires related education and relevant experience; competitive benefits available.

Qualifications

• Associate’s degree required.
• Bachelor’s or master’s degree preferred.
• Biohazard training required.
• Proficient in electronic health care record documentation and review.

Responsibilities

• Provide outreach and engagement services to unhoused individuals or at risk of homelessness.
• Coordinate a continuum of housing location, case management, and treatment services.
• Carry a caseload of clients with supportive counseling and concrete services.
• Complete documentation including Treatment Plans and IM+CANS in a timely manner per rules.
• Transport clients and conduct community visits.
