Job summary

LSSI is seeking a dedicated professional to provide outreach and engagement services for unhoused individuals with complex needs in the Chicago area. The role involves coordinating housing, case management, and treatment services, while maintaining accurate documentation and meeting regulatory requirements.

You will transport clients, support collaboration with recovery programs, and uphold ethical standards.

Qualifications

• Associate’s degree required.
• Bachelor’s or master’s degree preferred.
• Biohazard training required.
• Proficient in electronic health care record documentation and review.
• 1–3 years of outreach/engagement with unhoused individuals preferred.
• Experience with mental health and/or substance use population preferred.
• Familiarity with accreditation/licensure standards preferred.
• Strong communication, both verbal and written.

Responsibilities

• Provide outreach and engagement services to unhoused individuals or at risk of homelessness with serious mental illness and co-occurring substance use disorders.
• Coordinate a continuum of housing, case management, and treatment services.
• Manage caseload with supervisory guidance and collaborate with internal/external programs.
• Ensure timely documentation including Treatment Plans and IM+CANS, meeting applicable rules.
• Arrange transportation for clients and conduct community visits.
• Meet productivity standards (10+ billable hours per week).
• Adhere to NASW Code of Ethics and maintain professional conduct.
• Interact respectfully with clients, families, colleagues, and partner organizations.
