Responsibilities include coordinating with families, schools, and healthcare providers, and ensuring compliance with state mandates and agency policies.

The role emphasizes strength-based case management, advocacy, and collaboration with community resources.

Qualifications

• Bachelor’s degree in social work or related field required.
• State licensing/certification as required by Illinois DCFS.
• CWEL or ability to obtain required.
• CERAP required.
• Excellent communication in writing and speaking.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Complete and submit all required reports and documentation accurately and timely.
• Participate in supervisory process for guidance and decision making.
• Coordinate with schools and healthcare facilities to promote child well-being.
• Assess family service needs and link to community resources.
• Ensure child safety and well-being during service delivery.
• Provide strength-based case management and advocacy.
