Job summary

Pacific Clinics seeks a Cal-AIM Housing Navigator to ensure timely housing transitions, deposits, and tenancy supports for eligible members within the county.

The role collaborates with Continuum of Care, MCPs, and ECM providers to deliver housing services and uses HUD funding knowledge to support clients. Documentation in HMIS and Salesforce is required to track progress and outcomes.

Qualifications

• High school diploma or GED with 4+ years of relevant experience.
• Knowledge of housing rights for homelessness and HUD funding is preferred.
• Ability to work with diverse communities and maintain professional boundaries.
• Experience coordinating with housing and community resources.

Responsibilities

• Complete intakes and assessments to identify housing needs.
• Meet with clients weekly to monitor housing progression and plan.
• Identify subsidized housing options and assist with applications.
• Collaborate with landlords to develop housing stock and facilitate deposits.
• Maintain documentation in HMIS and Salesforce per HUD requirements.
• Educate clients on tenants' rights and landlord-tenant relations.
