Job summary

Equitable Social Solutions is seeking a dedicated professional to support clients experiencing homelessness in San Diego. The role focuses on conducting in-depth client interviews, assessing needs, and coordinating rapid housing referrals while maintaining detailed records.

The position requires strong communication, organizational skills, and the ability to collaborate with agencies, property owners, and service providers to advance stabilization and self-sufficiency for clients.

Qualifications

• Bachelor’s Degree in Human Services or equivalent experience.
• Intermediate computer skills in Microsoft Office, Email and Data Entry.
• Strong written and verbal communication skills.

Responsibilities

• Clearly present information to groups and explain services to clients, staff, and partners.
• Assess client needs and conduct in-depth interviews.
• Develop action and stabilization plans with clients and track progress.
• Manage referrals and coordinate housing options and services.
• Document expenditures and maintain records and files.
• Collaborate with community partners to support homelessness prevention and transition goals.
• Organize caseloads and prioritize tasks to meet deadlines.
• Provide counseling and supportive services to clients as needed.
