Job summary

City Care in Oklahoma City is seeking a highly organized Navigator to work with families and vulnerable adults on a flexible schedule. The role will involve case management, developing community resource networks, coordinating school enrollment for children, and promoting housing stability by documenting progress and coordinating service plans with partners.

Strong communication, data entry skills and the ability to navigate social services are essential.

Qualifications

• Previous experience in case management, recovery, or justice work; or work with adults and children with disabilities.
• Ability to use a computer and accurately enter data into a database; HMIS experience preferred.
• Pre-employment background check, driving record, and drug test.
• Current Oklahoma driver’s license, reliable transportation and updated car insurance.
• Preferred experience navigating mental/physical health systems, benefits and resources.
• Shelter experience or related recovery support background is a plus.

Responsibilities

• Carry out the mission, vision and values of City Care.
• Know the resources available in the community and develop a network of contacts.
• Coordinate school enrollment for children accompanying families.
• Develop individualized plans with families; identify self-directed goals and monitor progress.
• Provide case management, crisis intervention and direct support to families in shelter and community.
• Promote housing stability by documenting progress and linking resources in the record.
• Ensure ROI’s, case notes and weekly documentation are updated per best practices.
• Manage donation inventory in family suites and replenish stock with Volunteers.
• Exceed the minimum expectation of weekly face-to-face meetings with families.
• Facilitate in-house group meetings and community activities.
• Attend weekly staffing and community meetings; uphold confidentiality.
