Job summary

NorthCare in Oklahoma City seeks a Behavioral Health Case Manager II to provide outpatient care navigation for adults, children and families, ensuring a continuum of health and behavioral services with a recovery-focused, trauma-informed approach.

You will collaborate with a multi-disciplinary team, assist clients in obtaining benefits, housing, transportation, education or employment, and help clients set and reach personal health goals in their communities.

Qualifications

• CM II certification required and annual CE requirements.
• Wellness Coach Certification required and CE requirements.
• Minimum 2 years of CM II experience in community mental health.
• Community-based setting defined as any non-designated NorthCare Office.
• Must possess Oklahoma driver’s license during employment with NorthCare.
• Maintain automobile liability insurance including uninsured motorist coverage.
• CPR, First Aid and Therapeutic Options certifications required or to be obtained within 30 days.
• Co-Occurring Training and Trauma Informed Training within 60 days.
• TB test within 12 months prior to employment or within 30 days.
• CMII/PRSS dual certification preferred.

Responsibilities

• Provide integrated services that are welcoming, engaging, accessible and in the community.
• Promote a warm and engaging organizational culture in line with NorthCare values and trauma-informed care.
• Demonstrate clinical expertise in coordinated health and behavioral health care and mentor colleagues.
• Support clients in obtaining eligibility for benefits, housing, transportation, education or employment.
• Provide crisis management and safety planning as needed.
• Coordinate care across treatment settings and prepare for primary care appointments.
• Maintain professional competency and documentation per policy and accreditation standards.
• Travel to client homes and community settings as required.
• Maintain timely and accurate clinical records and meet productivity expectations.
