Job summary

PATH in California is hiring a Case Manager for the CalAIM Community Supports program in Los Angeles. This full-time, non-exempt role requires driving and offers an hourly wage of $23.00 to $29.00 with 24 pay periods per year.

The position focuses on intake, ISP development, care coordination, referrals, and ongoing assessment to support Medi-Cal beneficiaries experiencing homelessness and with complex health needs, aiming to improve health outcomes and housing stability.

Responsibilities

• Conduct intake and needs assessment and develop ISP addressing housing and other goals.
• Provide referrals and linkages to resources to support ISP goals.
• Completes program-specific assessments (e.g. VI-SPDAT).
• Evaluates progress with 90-day review and documents outcomes.
• Maintains complete participant files and adheres to PATH policies.
