Job summary

Qualifications

• Two years of experience in healthcare, case management, homeless services or working with vulnerable populations.

Responsibilities

• Conducts intake and needs assessment to develop ISP addressing housing, income and health goals.
• Provides referrals and support to resources to achieve ISP goals.
• Completes program assessments (e.g., VI-SPDAT).
• Evaluates progress with 90-day reviews and adjusts plans accordingly.
• Maintains accurate case files and documentation per PATH standards.
