Job summary

Responsibilities

• Conduct intake and develop individualized service plans addressing housing, income and personal goals.
• Provide referrals and linkages to resources to meet ISP goals.
• Complete program assessments (e.g. VI-SPDAT) and maintain documentation per PATH policies.
• Perform 90-day reviews and monitor progress toward housing stability.
• Maintain complete participant files with required documents.
