Job summary

Illumination Health + Home is seeking a Housing Case Manager to provide comprehensive case management for individuals experiencing homelessness or at risk. You will coordinate assessments, harm reduction strategies, trauma-informed care, and referrals to secure housing stability.

The role offers a daytime schedule, a wage of $22.00–$25.00 per hour, and opportunities to collaborate with an interdisciplinary team across multiple facilities in California.

Qualifications

• Required: Valid CA Driver’s License and eligibility for company vehicle insurance
• High school diploma or equivalent
• Professional verbal and written communication skills
• Access to a vehicle
• Proficiency in Microsoft Office/Teams (Mail, Docs, Sheets, Calendar)

Responsibilities

• Maintain a caseload of 30–35 clients
• Develop and execute individualized housing plans
• Engage with clients weekly
• Enter notes into EHR within 24–48 hours of engagement
• Connect clients with local resources and referrals
• Attend interdisciplinary team meetings weekly to track goals
