Job summary

Volunteers of America Northern California & Northern Nevada, Inc. is seeking a full-time Permanent Supportive Housing Case Manager to anchor on-site services at Pacific Crest Commons in Truckee, CA.

You will develop client-centered case management plans, linking tenants to education, employment, budgeting, and other supports. Housing First principles guide all services, with a goal of 85%+ housed at 12 months.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Minimum two years of social work-related education or three years of homelessness services experience required.
• Strong written and verbal communication skills required.

Responsibilities

• Develop a case management plan to assist PSH participants in achieving self-reliance and independence.
• Link participants to education, employment, budgeting strategies and related supports.
• Coordinate with internal and external care providers and maintain ISP and Crisis Response Plans.
• Conduct monthly house meetings and track progress toward 12-month housing stability goals.
• Maintain confidential client records and ensure HIPAA compliance.
