Job summary

Volunteers of America Northern California & Northern Nevada, Inc. is seeking a full-time Case Manager in Elk Grove to support transitional and permanent housing for residents experiencing homelessness.

You will develop individualized case management plans, connect participants to education, employment and budgeting resources, and maintain case files to document progress toward self-sufficiency. This role requires strong communication and collaboration with partners.

Qualifications

• Bachelor’s degree in Social Work or related field preferred.
• Minimum two years of social work-related education or experience with homeless populations.
• CPR/First Aid training within 90 days of employment.

Responsibilities

• Develop case management plans to increase self-reliance and housing stability.
• Link participants to education, employment, budgeting, and supportive services.
• Maintain case management files with goals, progress, and documentation.
• Maintain HMIS and Credible data, and ensure compliance with policies.
• Provide crisis management and mediation as needed.
• Coordinate with housing partners and service providers.
