Job summary

WestCare Inc seeks a Housing Navigator to identify and engage homeless individuals and families, building rapport and guiding them from street or shelter to interim housing and ultimately permanent housing.

The role focuses on connecting clients with income opportunities, health services, and durable housing solutions, while coordinating with FMCoc teams and maintaining regular progress updates.

Qualifications

• Excellent communication and interpersonal skills required.
• Strong documentation and computer skills.

• Ability to stay calm under pressure and de-escalate in the field.
• Knowledge of housing services and community resources is preferred.

Responsibilities

• Case management of a large caseload up to 30 clients.
• Identify housing resources and link clients to housing options.
• Assist with obtaining documents (ID, birth certificate, SSN, etc.).
• Provide transportation to services as needed.
• Offer referrals and advocacy to access services and resources.
• Assist with housing applications and paperwork; monitor progress.
• Ensure smooth transition to permanent housing and ongoing stability.
• Attend FMCoc trainings and participate in case conferencing.
