Job summary

WestCare Foundation in North Highlands, CA seeks a Housing Navigator to facilitate successful reentry by securing safe, stable, and affordable housing for participants after incarceration. The role collaborates with STOP staff, landlords, and housing authorities to identify suitable placements and address barriers to access.

The Housing Navigator develops discharge plans, assists with housing applications, and advocates with landlords to promote housing retention. Regional travel is required.

Qualifications

• Experience in case management or similar social service work.
• Knowledge of labor market research and vocational planning.
• Knowledge of offender populations and recovery concepts preferred.
• Excellent language and interpersonal skills.
• Ability to obtain and maintain CDCR clearance and maintain confidentiality.
• Valid driver’s license and ability to obtain WestCare vehicle insurance.

Responsibilities

• Assess housing needs, identify barriers, and develop housing-focused discharge plans.
• Develop transition and reentry plans addressing housing readiness.
• Connect participants to permanent housing options per eligibility.
• Provide follow-up and support to promote housing retention.
• Assist with housing applications and subsidized housing paperwork.
• Survey rental markets for affordable housing opportunities.
• Advocate with landlords, property managers, and housing providers.
• Maintain landlord database and document housing activity.
• Coordinate with STOP staff to align housing with case plans.
• Track housing outcomes for contract reporting.
