River Edge Behavioral Health in Fulton County, Georgia, is seeking a Full-time Care Coordinator for the Permanent Supportive Housing program. You will support housing applications, provide referrals for behavioral health and substance use services, and assist clients with housing needs in a client-centered environment.

The role requires a Master's degree in a social service field, two years of relevant experience, and a commitment to treatment planning and coordination of services.

Qualifications

• Master's degree in social service field (e.g., Social Work, Sociology, Psychology) or related field.
• Exemplify excellent communications skills, both verbal and written.

Responsibilities

• Work closely with the Case Manager to provide support to individuals living in permanent supportive housing.
• Maintain a caseload of up to 20 individuals and conduct in-home visits.
• Conduct confidential behavioral health assessments to determine level of care.
• Assist with development of solution-focused treatment plans.
• Participate in individual and group counseling when applicable.
• Maintain quality, up-to-date clinical records and discharge planning.
