Job summary

MWVCAA seeks a dedicated Case Manager to provide direct services to chronically homeless individuals, including eligibility screening, enrollment, and ongoing follow-ups. The role emphasizes Housing First initiatives, data entry, and collaboration with community partners.

Qualifying candidates may have a high school diploma or GED with two years in social services; preferred candidates possess a related college degree or specialized training and experience with homelessness services.

Qualifications

• High School Diploma or GED; two years of related social service experience or equivalent.
• Ideal candidates may have a college degree or specialized training with two+ years in homeless service delivery.
• Knowledge of coordinated entry processes and best practices in homeless service delivery preferred.

Responsibilities

• Facilitate distribution of rapid re-housing and barrier removal funds; ensure housing assessments available.
• Assess client eligibility and collect documentation for intake, enrollment, and financial assistance.
• Provide direct case management and housing placement support to help clients achieve self-sufficiency.
