Job summary

Cornerstones in Reston, VA invites an experienced Housing Focused Case Manager to coordinate services for individuals at the Embry Rucker Community Shelter (ERCS). Using a Housing First approach, you’ll help rapidly exit adult households from homelessness and develop personalized housing stability plans.

You’ll advocate for clients, connect with partners, and document outcomes in HMIS, while embracing trauma-informed, equity-centered practices and working with a dynamic Cornerstones team.

Qualifications

• Bachelor’s degree in an applicable human services field or commensurate experience with homeless and/or at-risk populations. Master’s Degree preferred.
• MSW plus two years post-Master’s degree a plus.
• Bi-lingual in English/Spanish, English/Arabic, English/French is preferred.
• Proficiency with Microsoft Office suite or related software.

Responsibilities

• Manages an average caseload of 12-15 adult only households experiencing homelessness at ERCS.
• Conducts comprehensive in-depth assessments utilizing standardized tools and action planning.
• Develops and monitors individualized housing stability plans with goals and steps.
• Educates shelter participants about obtaining affordable housing with Housing First principles.
• Advocates for services and connects participants to benefits and housing resources.
• Maintains HMIS case records and documents outcomes and progress.
