Job summary

Colorado Coalition for the Homeless is seeking a Case Manager to provide coordinated stabilization and time-limited case management to clients exiting unsheltered homelessness or transitional shelter sites in Denver, CO.

You will work with a collaborative team including behavioral health navigators, housing navigators and other service providers to ensure rapid exits into housing and follow-up care for at least the first year.

Qualifications

• High School Diploma or GED required.
• Bachelor's degree preferred in Psychology, Social Work or Human Services.
• Basic Life Support Certification required within 30 days of employment.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Maintain positive relationships with community partners and health providers.
• Works under direct supervision, with department parameters and team collaboration.
• Support clients with a wrap-around care management model to ensure engagement and supports during crisis or disengagement.
• Acts as a liaison between internal program management, client referrals and community partners.
• Responds promptly to crises involving residents with de-escalation and referrals.
