Job summary

The First Presbyterian Church of Dallas is seeking a RRH Case Manager to support the Street to Home Initiative, collaborating with Housing Forward and other partners to end homelessness. You will provide case management to individuals and families who are homeless or at risk, helping them find and maintain housing and achieve independence.

This grant-funded role requires field-based work, ongoing housing stability, income acquisition, and self-determination for participants.

Qualifications

• Degree: Bachelor’s in social work or related field; Master’s preferred.
• 2+ years in case management with vulnerable populations.
• Experience with housing is preferred.
• Reliable transportation, valid license, active auto insurance.
• Proficient in Word, Excel, and Outlook.

Responsibilities

• Maintain a caseload of 27 households in Street to Home Initiative.
• Complete eligibility assessment for new participants and ensure funding guidelines.
• Develop strengths-based, goal-driven housing plans with participants.
• Assist participants to obtain income, education/training, and treatment as needed.
• Teach budgeting basics, bill payment, and savings concepts.
• Build referral networks for housing retention and services.
• Use motivational interviewing, trauma-informed care, and harm reduction.
• Conduct monthly and annual rent/income/asset assessments.
• Assist with housing applications, landlord negotiations, and lease review.
• Cultivate relationships with landlords and property managers.
• Coordinate furniture, welcome baskets, and unit readiness for move-ins.
• Perform monthly home visits and regular office visits.
• Document case management activities in HMIS accurately and timely.
• Enroll/update/exit participants in HMIS; maintain housing participant spreadsheets.
