Job summary

Step Up in Nashville is seeking a Housing Case Manager to provide intensive case management, engagement, and stabilization services to adults facing chronic homelessness in scattered site housing. The role supports Housing First, Harm Reduction, trauma-informed care, and Motivational Interviewing to promote housing stability, linkages to services, and recovery‑oriented outcomes in the Nashville community.

You will partner with a new Housing First team to expand Step Up’s permanent housing model

Qualifications

• Minimum of 2 years related housing/case management with chronically homeless adults.
• Knowledge of housing vouchers and field-based service delivery.
• Experience documenting services using BIRP notes format.

Responsibilities

• Provide intensive Housing First case management and engagement to support permanent housing.
• Meet residents through home visits, review goals, and build vocational skills.
• Coordinate care and link to community resources for independent living.
• Monitor for crisis signs and collaborate with the team.
• Document services using required record-keeping systems.
