Job summary

UMOM New Day Centers in Phoenix, AZ, is seeking a Case Manager to engage clients, provide direct case management, and advocate toward permanent housing, with a focus on skill building and resource connections.

You will work with the workforce development team, document progress in HMIS/ETO, and coordinate with community providers. A high school diploma is required; a bachelor's degree is preferred, and at least 3 years of experience with at-risk populations.

Qualifications

• High School or GED diploma required.
• Bachelor's degree preferred.
• Minimum 3 years of experience working with at-risk populations and/or case management.
• Knowledge of Motivational Interviewing and Trauma-Informed Care preferred.
• Knowledge of nonprofit sector; experience with underserved populations preferred.
• Knowledge of homeless services, workforce and housing providers.

Responsibilities

• Engage clients to build rapport and provide direct case management and advocacy.
• Coordinate with Workforce Development and outside providers.
• Enter timely and accurate case notes and assessments; maintain confidentiality.
• Assist clients in securing identification and housing documentation.
• Develop and review case plans to achieve permanent housing and employment goals.
