Job summary

UMOM New Day Centers in Phoenix, AZ, is hiring a SSVF Case Manager to deliver housing-focused case management to Veterans and their families in the SSVF program. You will identify housing barriers, develop individualized plans, connect participants with income resources, and coordinate with landlords and service providers to promote long-term housing stability.

Exempt, full-time role requires at least a high school diploma and 3 years of related experience; a Bachelor's degree is preferred.

Qualifications

• Must have a High School diploma or GED.
• Bachelor's degree preferred.
• Minimum 3 years of experience with at-risk populations and/or case management.
• Knowledge of Motivational Interviewing and Trauma Informed Care preferred.
• Knowledge of nonprofit sector; experience with homeless services and housing resources preferred.

Responsibilities

• Engage Veterans and families with client-centered case management; build rapport and collaboration.
• Develop individualized housing plans and address barriers to permanent housing; coordinate with landlords and service providers.
• Maintain accurate case records, assessments, and documentation in line with program and agency standards.
