Job summary

The Salvation Army USA Western Territory is seeking a dedicated Case Manager to provide comprehensive case management for Booth Senior Bridge Home residents and Chester Village families in Glendale, CA. You will develop housing plans, coordinate external resources, and maintain detailed documentation to ensure compliance with program contracts.

Responsibilities include intake, assessment, ongoing monitoring, and collaboration with mental health workers to support stability.

Qualifications

• BA in human services with 2+ years in social services or 4 years case management.
• Valid California driver's license and pass a criminal background check.
• Ability to keep client information confidential and manage data accurately.
• CPR/First Aid Certification.

Responsibilities

• Maintain accurate case notes and data entry for housing contracts and monthly reports.
• Coordinate referrals and external resources for residents and families.
• Collaborate with DMH and partner agencies to support mental health and housing plans.
• Conduct intake, assessment, service planning and ongoing progress monitoring.
• Develop short- and long-term goals with clients and track outcomes.
• Prepare progress notes and ensure timely documentation for contract requirements.
