Job summary

The Salvation Army Glendale Corps is seeking a dedicated Case Manager to provide comprehensive case management for residents and families within Booth Senior Bridge Home and Chester Village programs. You will coordinate housing assessments, resources, and referrals to support stable housing and wellness outcomes.

Responsibilities include documenting progress, data entry into HMIS, and regular reporting to LAHSA and project partners.

Qualifications

• BA in human services with 2+ years case management experience.
• Valid California driver’s license and criminal background check.
• Confidentiality and professional conduct.
• Computer literacy and ability to use computerized database systems.

Responsibilities

• Maintain documentation per program contracts, including progress notes and reports.
• Enter CES data into HMIS and keep organized client files.
• Prepare periodic reports for LAHSA, HIMIS/Clarity and contracted entities.
• Collaborate with DMH staff to support mental health engagement.
• Provide case management for families and residents, including intake and ongoing monitoring.
• Develop short and long-term goals with clients and reassess needs.
