Job summary

U.S.VETS in Prescott, AZ is hiring a Case Manager II to provide comprehensive support to veterans facing housing instability, trauma, and mental health challenges. You will perform assessments, develop service plans, and coordinate with the VA, landlords, employers, and community resources.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field.
• At least two years of work experience in case management or social services.
• Ability to pass background checks for programs involving incarcerated/recently released clients.
• Strong communication, documentation, and organizational skills.
• Proficiency in Microsoft Office and electronic records systems.
• Valid driver's license with clean driving record.
• Experience with homeless or veteran populations a plus.
• Substance Abuse Counseling certification (e.g., CCAPP, CAADE) a plus.
• Experience facilitating groups or life skills classes.

Responsibilities

• Conduct client assessments and develop individualized service plans addressing housing, employment, education, recovery, legal, financial, medical, and mental health needs.
• Coordinate and monitor services with the VA, landlords, employers, treatment providers, and community resources.
• Provide regular one-on-one meetings with clients and conduct home visits.
• Transport clients to appointments and meetings as needed.
• Facilitate life skills or therapeutic groups for veteran populations.
• Assist with crisis intervention and after-hours support as needed.
• Document services and reports in electronic records systems in a timely, professional manner.
• Support reporting, audits, and file reviews with Program Coordinator and Director of Behavioral Health.
