Job summary

Southwest Behavioral and Health Services is seeking a Case Manager for Supported Housing in Flagstaff, AZ. You will coordinate with residents, manage leases, and ensure housing compliance while applying HUD and ADA guidelines.

The role emphasizes recovery-driven care, housing stability, and collaborative work with internal and external partners. A Bachelor’s degree in a related field is required along with background checks.

Qualifications

• Bachelor’s degree in social/behavioral sciences or related field.
• Equivalency: a total of five years of related integrated health experience accepted.
• Housing or case management experience with underserved populations preferred.
• Negative TB results required.
• Background check (39-month MVR) and fingerprint clearance may be required.
• May drive a personal vehicle for program needs.

Responsibilities

• Perform intakes, screenings and assessments for potential residents.
• Manage leases, notices, and lease compliance to promote housing stability.
• Coordinate housing procedures per HUD regs and inspect unit habitability.
• Provide housing services per Fair Housing Act and ADA accommodations.
• Oversee caseload, quarterly housing plans and inspections; manage work orders.
• Meet residents, assess needs and coordinate crisis supports as needed.
• Document services and complete background checks; maintain charts.
