Job summary

Southwest Behavioral and Health Services is seeking a dedicated Case Manager for Supported Housing in Kingman, AZ. The role focuses on intake, housing stability, and coordination with landlords and service providers to support residents in integrated community living.

Qualifications

• Bachelor's degree in a related field with 1 year of integrated health experience.
• Equivalent education/experience in integrated health acceptable.
• Housing or case management experience preferred.

Responsibilities

• Performs intakes, screening and assessments on potential residents.
• Manages lease processes, monitors compliance and promotes housing stability.
• Coordinates notices of noncompliance and eviction procedures with the Program Manager.
• Inspects units for HQS/Habitability and ensures HUD compliance.
• Documents services and maintains consumer charts and eligibility.
