Job summary

Southwest Behavioral & Health Services is seeking a Case Manager for Supported Housing in Prescott Valley. You will provide housing support services to help residents participate in their communities, applying recovery-focused principles to improve quality of life and independence.

Responsibilities include intake, housing coordination, lease management, housing inspections, and crisis support, with emphasis on HUD compliance and fair housing practices.

Qualifications

• Bachelor’s Degree in Social/Behavioral Sciences, Public Administration, Business, Urban Planning or related field with 1-year full-time integrated health experience.
• Any equivalent combination of education and experience directly related to integrated health totaling five years is acceptable.
• Housing or case management experience with underserved or vulnerable populations preferred.
• Negative TB results required.
• Fingerprint clearance issued by the Arizona DPS or eligible to receive upon hire.

Responsibilities

• Performs intakes, screening and assessments on potential residents; handles lease processes and monitors lease compliance.
• Coordinates notices of noncompliance, eviction documentation, terminations and relocations.
• Ensures HUD housing regulations, Fair Market Rents, Rent Reasonableness and HQS/Habitability standards are followed.
• Provides housing per Fair Housing Act and ADA reasonable accommodations.
• Oversees caseload and supports service delivery; completes Housing Plans and inspections; tracks repairs and turnovers.
• Meets with residents and supports stabilization of housing; inspects units and safety equipment; coordinates renewals and repairs.
• Coordinates referrals, follows up on unmet resident needs; liaises with landlords and service providers.
