Qualifications

• Bachelor’s degree or equivalent with required experience.
• High School Diploma or GED required.
• Valid CA driver’s license and current auto insurance per contract requirements required.
• Knowledge of housing needs of individuals with behavioral health needs preferred.

Responsibilities

• Conduct weekly housing inspections and address client-reported issues.
• Provide case consultation, crisis intervention, discharge placement, and targeted housing case management.
• Complete documentation as required and support billable services.
• Locate housing opportunities and manage lease agreements.
• Prepare subsidy applications and maintain client housing files.
• Assist in rent collection and help clients obtain required documents.
• Conduct housing screenings and assessments using the coordinated entry system.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
