Rosecrance Behavioral Health in Rockford, Illinois is seeking a Supported Housing Case Manager to work within the community providing mental health treatment and support. This full-time position requires a Bachelor’s Degree in a health-related field and involves working closely with clients to foster independence and stability.
The candidate will participate as a team member in mental health service delivery, requiring strong organizational and communication skills. The role includes case management and weekly community support services.
The Supported Housing Case Manager works with clients residing in the Supported Housing buildings to provide behavioral health treatment in the client's natural setting. The case manager is responsible for completing comprehensive assessments, developing an individual service plan, and monitoring progress. The case manager provides weekly case management and community support services both on site and within the community, assists clients with fostering independence, maintaining safety and stability while maintaining housing and benefits, and improving self‑determination through illness management. The case manager coordinates care with the client, family/natural supports, and outside providers. The staff attends weekly client staffing, monthly meetings with the program managers, and monitors and reports on performance indicators and grant deliverables.

$3,000 sign‑on for non‑licensed
Tuesday–Saturday 8:30 a.m. – 5:00 p.m.
