Job description

Position Summary

The Supported Housing Case Manager works with clients residing in the Supported Housing buildings to provide behavioral health treatment in the client's natural setting. The case manager is responsible for completing comprehensive assessments, developing an individual service plan, and monitoring progress. The case manager provides weekly case management and community support services both on site and within the community, assists clients with fostering independence, maintaining safety and stability while maintaining housing and benefits, and improving self‑determination through illness management. The case manager coordinates care with the client, family/natural supports, and outside providers. The staff attends weekly client staffing, monthly meetings with the program managers, and monitors and reports on performance indicators and grant deliverables.

Job Requirements
  • Bachelor’s Degree in a health‑related field, two years of experience in mental health preferred
  • Valid driver’s license and reliable transportation
Qualifications
  • Demonstrated education, training, or experience in mental health services and/or co‑occurring disorders
  • Adequate written and computer skills to accurately complete required documentation within the prescribed time frames
  • Excellent organizational and computer skills
  • Self‑starter with ability to work independently, with a team, and in the community
  • Skilled in conducting individual, family, and group counseling, case management
  • Ability to provide education and support to clients, families, staff, or community resources
  • Open and effective line of communication with the team
  • Able to perform or learn de‑escalation techniques
  • Work effectively and professionally with community agencies
  • Comfortable working independently in the community and within a client’s home
  • Establish a healthy work‑life balance
  • Forget preconceived notions of mental illness
Essential Responsibilities
  • Provide mental health rehabilitation services and supports to adults and families to decrease hospitalizations and crisis episodes and to increase community functioning in order for the client to achieve rehabilitative, resiliency, and recovery goals
  • Provide continuity of program services in an ethical, legal, and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards
  • Participate as a team member in the delivery of mental health services to clients and their families and facilitate the integration of recovery into their daily living situations
  • Provide case management services, crisis intervention, skills training, outreach, and group services
  • Complete documentation within the required time frame
  • Attend meetings
  • Ensure quality improvement
  • Complete other duties necessary to meet the needs of the seriously mentally ill and homeless population

Sign‑on Bonus

$3,000 sign‑on for non‑licensed

Schedule

Tues­day–Sat­ur­day 8:30 a.m. – 5:00 p.m.

Pay

Bachelor’s starting at $46,800
