Job summary

PATH is seeking a Case Manager in Los Angeles, CA to support clients through all homelessness stages, from street outreach to permanent housing. You will help access health and social services, assist with benefits, and develop individualized care plans focused on housing and health outcomes.

The role emphasizes trauma-informed care, motivational interviewing, and collaboration with partners to create effective support networks. Driving is required and eligibility checks apply.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals and linkages to resources to meet ISP goals.
• Complete program assessments (e.g. VI-SPDAT).
• Evaluate progress with 90-day reviews of the participant.
• Maintain accurate case files and documentation per PATH policies.
• Ensure confidentiality of participant files per HIPAA and guidelines.
• Develop therapeutic relationships to support housing goals.
• Provide crisis interventions and risk assessments to enhance safety.
• Use motivational interviewing and trauma-informed care in service delivery.
• Keep knowledge of community resources up to date to provide wrap-around services.
