Housing Works is seeking a Clinical Care Manager for the BEW Project, a partnership with NYC DOHMH, to support Black and Latina women living with HIV in reengaging in care and essential support services. You will assess client needs, coordinate care, develop health goals, and promote retention in care with an interdisciplinary team.

Applicants should have MSW/LMSW or BA/BS with 5+ years of care management, be culturally competent with diverse gender identities, and be able to document in

• Master Social Worker (MSW/LMSW) or BA/BS with at least 5 years care management experience.
• Cultural competency with Black and Latina communities and diverse gender identities.
• Experience completing assessments and developing care plan goals.

Responsabilidades

• Facilitate BEW program enrollment including intake, outreach and eligibility verification.
• Conduct initial needs assessments and collaborate with clients on care goals.
• Reassess and update plans of care at regular intervals.
• Verify engagement in HIV treatment and timely reconnection to care.
• Collaborate with interdisciplinary team on structured health education sessions.
• SupportClient Navigator in delivering enhanced navigation services to access care and services.
