Job summary

BRC is seeking a dedicated Case Manager to support chronically homeless individuals in New York, NY. The role involves intensive case management, regular client contact, and coordination with clinical staff to overcome housing barriers and secure entitlements.

You will develop individualized housing plans and connect clients with a network of services. The position emphasizes strong communication, documentation, and collaboration, with a commitment to helping clients progress toward stable

Qualifications

• High School Diploma/GED required; bachelors degree preferred and masters ideal.
• Bilingual Spanish strongly preferred, but not required.
• Excellent communication and writing skills and computer proficiency required.

Responsibilities

• Provide comprehensive case management services to identified chronic homeless individuals.
• Meet with clients regularly both in the field and in the office; maintain complete and up-to-date clinical files and reports.
• Conduct comprehensive assessments to identify barriers to housing and assist clients to secure entitlements.
• Develop and work with clients on individualized housing plans and build a network of support services.
